Research Request

If you are an Ophthalmologist or Optometrist registered with the Ohio Amblyope Registry and would like to receive information on amblyopia research currently being undertaken in Ohio, please submit the following form, below, and we will e-mail you the amblyopia research file. The amblyopia research file is composed of amblyopia research notices submitted to the Ohio Amblyope Registry. In order to be included in the amblyopia research file, the research protocol must have been reviewed and approved by a human subjects Institutional Review Board (IRB). Before research information is included in the amblyopia research file, the Principle Investigator (PI) must submit proof, in writing, that the amblyopia research protocol has been reviewed and approved by their respective IRB. Private office based amblyopia research, that has not been reviewed/approved by an IRB, will not be considered for inclusion into the amblyopia research file.

If you are currently undertaking amblyopia research and would like an e-mail notice of that research included in the amblyopia research file (and sent to Ophthalmologists and Optometrists in Ohio who request such information), please submit written IRB confirmation that the study has been approved by an IRB to the Ohio Amblyope Registry (see address below, submission form) AND e-mail the notice about the study that you would like included in the amblyopia research file. Please limit such notices to no more than two pages.
